Eligibility Criteria: 1. Attend a designated A+ high school for three (3) years immediately prior to graduation. At a minimum the last semester of grade must be given by Linn R-II High School. 2. Have a grade point average of 2.5 or higher on a 4.0 scale. 3. Have at least a 95 percent attendance record over a four-year period. Any student appropriately counted for average daily attendance will be considered as in attendance, whether physically present or not 4. Perform at least 50 hours of unpaid tutoring or mentoring services to youth. 5. Maintain a record of good citizenship and avoid the unlawful use of alcohol and drugs, as defined by School District policy. 6. Addendum requirements State a. Male students must complete the U.S. Military Selective Service registration as required by law. b. Students wishing to receive benefits through the A+ Schools Program must complete a Free Application for Federal Student Aid (FAFSA) before April 1st of the year they graduate. c. The student will be asked to provide a Social Security number as part of the A+ application process. The student will be informed that Department of Elementary and Secondary Education uses the number for the purposes of determining eligibility to receive A+ Tuition Incentive funding and to make payment to the public career center or two year college the student attends after being designated an A+ graduate. 7. Addendum requirements Osage County R-II School District a. Perform at least 25 hours of volunteer services that benefit the school and/or community. b. Participating students and a parent must each school year after the initial year an A+ School Participation Agreement is signed sign a renewal of the agreement. c. Participating students and a parent must each school year after the initial year an A+ School Citizenship Affidavit is signed sign a renewal of the affidavit. d. Never in any single school year have any combination of Out of School and In School Suspensions with the number of occurrences equal to or greater than 5. e. Must provide documentation that all eligibility criteria have been met by April 20th of the year of their graduation A final contract must be signed by both the participating student and a parent giving the School District permission to release information required by the State for their certification no later than May 1st of the year of their graduation.Related Internet Links:
