Parent Portal Information
Any updated Immunization Information not included in records from transferring school.

Office must have complete records from transferring school before student can be enrolled.

Official Administrative Record (name, address, birthdate, grade level completed, grades and attendance.
Transcripts/Grades
Withdrawal Grades & Percentages
Standardized Achievement Test Scores/Intelligence and Aptitude Test Scores
End of Course Test Scores
Health Records
Psychological Reports/Special Education Information/IEP/Evaluations
Discipline Records
Constitution Test Results (State and U.S.)
A+ School Program Information
Legal/Custody Documents (if applicable)
Any other pertinent information regarding student
